New Patient Registration

The doctors welcome new patients who live within our practice area.

Catchment Area

When registering we will ask you to complete a brief questionnaire for each member of the family. It is very important that you complete this as it may take a while for your medical records to reach us.

We would also strongly encourage you to have a New Patient Medical with one of our nurses/healthcare assistant so that we are aware of your current health status. New patients who are already taking medication will need to see the doctor before a repeat prescription can be issued.

All new patients are required to provide proof of identity, eg. driving licence, passport, council tax bill.

When you register, it will be with the practice and although you will be allocated a specific doctor you are welcome to see any doctor.

Non-English Speakers
